Licensed Dietitian – Mission-Driven Role Supporting Older Adults

HealthCare Support is seeking a Licensed Dietitian (LD) to join a compassionate and collaborative Program of All-Inclusive Care for the Elderly team dedicated to improving the health and quality of life for older adults in Springfield, OR.

This role offers the opportunity to function as a key member of an Interdisciplinary Team, working closely with participants, families, and healthcare providers to deliver personalized nutrition care. The LD will support individuals with complex medical needs while honoring participant preferences and goals, making a meaningful impact every day through education, care planning, and dietary program coordination.

Schedule: Part-time | 8:00am - 12:00pm

Compensation: $36 - $40/hour

Daily Responsibilities For The Licensed Dietitian (LD)

• Conduct comprehensive nutritional assessments across care settings, including occasional home and community visits
• Actively participate in IDT meetings, contributing clinical expertise in care planning and coordination
• Develop and implement individualized nutrition care plans based on participant needs, preferences, and medical conditions
• Provide nutritional counseling and education to participants, families, and caregivers
• Support management of chronic conditions including diabetes, heart failure, renal disease, and fluid/blood glucose balance
• Promote wellness, healthy eating habits, and self-management strategies
• Inspect meals for compliance with prescribed diets, quality, and presentation standards
• Coordinate menu planning, recipe development, and collaboration with food vendors
• Work closely with physicians, nurses, social workers, therapists, and other healthcare professionals to deliver integrated care
• Document all services accurately and timely within the Electronic Health Record

Required Qualifications For The Licensed Dietitian (LD)

• Active Licensed Dietitian (LD) or Registered Dietitian (RD) credential
• Knowledge of chronic disease management and medical nutrition therapy
• Ability to travel locally for participant visits as needed
