Position Summary:

Vitae Health Systems is seeking a compassionate and skilled Physician Assistant to provide comprehensive, patient-centered medical care to residents in skilled nursing facilities. The Physician Assistant will assess, diagnose, and manage acute and chronic health conditions, working collaboratively with an interdisciplinary team to enhance patient health and quality of life.

Key Responsibilities:

  • Conduct thorough health assessments, diagnose and manage acute and chronic conditions, and develop individualized, evidence-based care plans for residents in skilled nursing settings.
  • Order and interpret diagnostic tests, prescribe treatments, perform medication reviews, and monitor patient progress to prevent complications and hospitalizations.
  • Provide timely clinical interventions in response to changes in patient status to ensure high-quality, patient-centered care.
  • Support value-based care initiatives by focusing on quality outcomes, reducing avoidable hospital readmissions, optimizing cost of care, and using data to guide clinical decisions and improve performance.
  • Collaborate with physicians and interdisciplinary team members to ensure coordinated and efficient care, participating in care plan meetings and quality improvement efforts.
  • Facilitate smooth transitions of care across different settings, ensuring continuity and minimizing gaps in treatment.
  • Educate residents and their families about diagnoses, treatment plans, medications, and preventative care strategies to promote engagement and better health outcomes.
  • Maintain accurate, timely, and compliant documentation adhering to all clinical, regulatory, and organizational standards.

Requirements

  • Master’s degree from an accredited Physician Assistant program.
  • Current and unrestricted Physician Assistant license in the respective state.
  • Certification by the National Commission on Certification of Physician Assistants (NCCPA).
  • Minimum of 2 years clinical experience preferred, especially in geriatrics, long-term care, or post-acute care settings.
  • Strong clinical assessment, diagnostic, and treatment planning skills.
  • Knowledge of chronic disease management and geriatric care principles.
  • Commitment to delivering high-quality, value-based care focused on outcomes, cost-efficiency, and hospitalization reduction.
  • Proficiency in evidence-based clinical practice and decision-making.
  • Ability to work independently and collaboratively within an interdisciplinary team.
  • Excellent communication and interpersonal skills with residents, families, and healthcare staff.
  • Effective organizational, time management, and documentation skills.
  • Familiarity with electronic health record (EHR) systems.
