Home Care by Callos , is a non -skilled Home Care Agency currently recruiting a Registered Nurse professional for the position of Admissions Nurse . THERE IS NO PATIENT CARE INVOLVED IN THIS ROLE. Must be available Mon-Fri for Day shifts. This position is ideal for a dynamic, self-motivated professional who enjoys being on-the-go and has a passion for making meaningful connections. The Community Liaison will be responsible for building strong relationships with healthcare providers, community partners, and referral sources to promote our Home Health services.

Responsibilities of the position include:

• Educate healthcare professionals, facilities, physicians, and community organizations on the benefits and services of Home Care by Callos
• Perform comprehensive nursing assessments for admissions, readmissions, and start of care visits.
• Build and maintain strong professional relationships with Clients and Families.
• Ensure compliance with all regulatory requirements and company policies.
• Must be able to work with waiver/ private pay patients and CareScout clients
• Paid per visit/ plus mileage
• Develop and implement individualized care plans based on patient needs.
• Complete Carescout assessments accurately and timely.
• Remain active and on the go throughout the day educating patients/conducting training, and community partners.

Qualifications / Experience Requirements:

Must have:

• State Licensed RN
• Experience in a Homecare setting preferred.
• Experience working with elderly patients
• Excellent written and verbal communication skills
• Must be tech savvy
• Strong interpersonal, communication, and relationship-building skills.
• Self-starter with excellent organizational and time -management abilities.
• Ability to work independently while being part of a collaborative team
• Valid drivers license, reliable transportation, and current auto insurance
